About Us
We are committed to helping create a world where anyone can heal or age at home. We too are sons, daughters, mothers, fathers and caregivers. We do what’s necessary to keep our families safe and healthy. Unfortunately, most of us have experienced times in our lives when loved ones have been hospitalized. We yearn for when they will return home, yet are uneasy about coordinating all of their care and the associated costs.
CareCentrix is passionately focused on home health solutions and assisting as many patients as possible in receiving the quality care they need, surrounded by loved ones, in the comfort of their homes. Many patients prefer to recover at home, where they have the peace of mind of being around loved ones while healing in a familiar space where they feel most comfortable.
Company History
Since 1996, CareCentrix has developed a home health care model that integrates patients, clinicians, hospitals, health insurance plans and home care providers by streamlining and organizing the care management process from post-acute recovery in the hospital to the home.
CareCentrix improves access, care and engagement by:
providing access to quality home care through a national, credentialed network of home health providers
working with providers and patients to coordinate quality care in the most appropriate location – the patient’s home – as much as possible
providing patient interventions and educational programs, thereby assisting high risk and chronic disease patients in transitioning from the hospital to the home
Our approach to home health care is grounded in our values. We are passionate about making measureable contributions to improving outcomes for our patients while lowering costs. Our role is to help everyone in the home health system work more effectively and seamlessly together to positively impact the patient’s health care experience. CareCentrix provides services for its patients in the same way we hope others would assist our own loved ones.

PTO
You can use PTO for vacation, personal time, and sick time after 90 days of continuous service. In your first year of employment, PTO is based on hire date. At the first calendar year after employment, we offer 20 PTO days per year. After 5 years of service, you receive 25 PTO days and after 10 years, 30 PTO days!
Birthday
Our team members receive an additional paid day off to celebrate their birthday!
Appreciation Day
After one year of continuous service, our team members receive an additional paid day off to pursue personal activities of their choice!
